State policy and collective bargaining agreements require that employees be provided an annual performance evaluation. This evaluation presupposes that each position has a position description that identifies the specific duties of the position and that written measurable objectives have been agreed to.
The performance assessment is to be a growth-producing, continuous improvement process so must be completed in consultation with the employee. The Performance Review template is available on this website.
During the performance assessment process, the employee's position description should be reviewed to assure it is accurate and current. Guidelines prescribe that job descriptions over three year old must be revised. If you find the position description needs revision, or is over three years old, it should be rewritten. An important part of the performance assessment process is the development of a professional development/training plan. Together with the employee, identify professional growth opportunities and any necessary training the employee may require to be successful or stay current in their job. Note that priority in the allocation and approval of staff development funds is given to employees who have a professional development/training plan on file. The supervisor is to keep the original of the Individual Development Planning Worksheet and update/revise it as necessary throughout the next performance assessment cycle as additional professional development/training activities are identified and staff development funds are requested for the employee. The final, completed form should be submitted at the end of the performance assessment cycle. The Indivudal Professional Development/Training template is avaiable on this website.
At the conclusion of the process, the signed/dates and completed Performance Review, Individual Development Planning Worksheet and Position Description (if revised) forms should be submitted to the Human Resources Office for inclusion in the employee's official personnel file.
