Bias Incident Reporting Form

Use this form if you have been the target of a bias incident or have witnessed a bias incident. A bias incident is an act of bigotry, harassment, or intimidation that is motivated in whole or in part by bias based on an individual’s or group’s actual or perceived race, color, creed, religion, national origin, gender, age, marital status, disability, public assistance status, veteran status, sexual orientation, gender identity, or gender expression.

CARE Team Referral Form

Use this form to report students who may be experiencing distress or whose behaviors have raised concern for their well-being. The CARE Team will follow up with the student to provide timely and supportive assistance.
