Employees involved in or who observed an accident involving another employee MUST contact Human Resources to complete the appropriate forms.
Students, student employees and visitors/guests involved in or who observed an accident, please fill out a STATE OF MINNESOTA INCIDENT REPORT and return it to Campus Safety.
